ABOUT PROJECT

As part of a collaborative team with Johns Hopkins School of Nursing, this project applies communication design principles to visualize pathways for CAPABLE—a national non-profit program helping elderly individuals age safely in their homes to scale in a sustainable manner.
CAPABLE is a evidence-based program that combines occupational therapy, client-centered nursing, home safety assessments, and handyman services in an integrated approach to elder care and helping them age in place.

RESPONSIBILITIES

Conducted secondary research, primary research-interviewed stakeholders across the implementation spectrum, including subject matter experts, site implementors, and training specialists.

Further through iterative diagramming aided in developing comprehensive ecosystem model visualizing CAPABLE's operational structure, which were then adapted to reflect the unique characteristics of various local implementation sites.
